Under supervision of the Program Manager, the Lead Care Manager will act as Team Lead for non-clinical Care Managers within assigned counties. The Lead Care Manager is responsible for providing support to assigned Care Managers including but not limited to initial precepting of new Care Managers, quarterly Field Visits, day-to-day escalations, chart audits, productivity tracking, and providing direct member outreach and casework support as needed. Lead Care Manager responsible for attending Collaborative Planning and Implementation (CPI) meetings for assigned counties. Lead Care Manager responsible for initiating and maintaining working relationships with county managed Medi-Cal plans and community referral sources.
How will I contribute
Plan, organize and direct the work of Care Managers in assigned counties. Complete chart audits for Care Managers in assigned counties and follow-up on incomplete documentation. Act as first point of escalation for Care Managers in assigned counties. Provide initial field precepting to new Care Managers in assigned counties. Complete quarterly Field Visits for Care Managers in assigned counties. Attend Collaborative Planning and Implementation (CPI) meetings for assigned counties. Initiate and maintain working relationships with community referral sources and managed Medi-Cal plans in assigned counties. Provide direct member outreach and casework support as needed. Other duties as assigned.
What I bring
High School Diploma plus 1 year of ECM/CS specific experience or B.A. degree for human services area of study Class C Driving License Knowledge of behavioral health, criminal justice system, and housing/homelessness, including challenges, needs, resources, interventions, and/or other content knowledge. Ability to prepare and conduct care management assessments and plans, communicate effectively with groups and individuals both verbally and written, instruct and assist in obtaining services and information. Develop program material and use effectively, establish, and maintain working relationships with a variety of agencies, organizations, and individuals. Able to work in inclement weather and in unsanitary conditions as needed. BA or BS preferred. Bilingual English/Spanish preferred.
