Community Engagement Program Care Coordinator (Project Management)
Evaluates social determinants of health, identifies barriers to care and addresses/resolves obstacles impeding progress
Provides support to help maximize quality measures
Assist client with meeting the goals identified in their care plan
Determines the appropriate level of care coordination needed to support the patient at any given point in time
Facilitates patient access to appropriate services, both internal and external
Follows up on no-show patients and fill same day cancellation appointment slots.
Participates in huddles, team meetings, and case conferencing as needed.
Educates patient and family/caregiver(s) about relevant community resources
Acts as point of contact for any behavioral health needs
Collaborates with general care coordinators to limit overlap and ensure closed-loop BH referrals
Provides support to behavioral health providers by completing forms and other tasks as needed.
Administers new and returning patient self-report and satisfaction questionnaires
Documents timely and efficiently in the EHR
Attends appropriate training courses/webinars and meetings
Gathers and reports data related to aspects of care coordination monthly
Provides feedback for the improvement of Care Coordination processes
Perform other duties as assigned
KNOWLEDGE, SKILLS, AND ABILITIES
Proficiency with Microsoft Office applications including Outlook, Word, Excel and PowerPoint
Ability to prioritize projects, work relatively independently, manage multiple tasks, and meet deadlines
Strong organizational skills
Strong written and verbal communication skills
Ability to effectively present information, respond to questions, and generally communicate clearly and respectfully with participants, coworkers, other agencies and companies
Comfortable discussing sensitive health issues with clients
Knowledge of care coordination best practices
Strong collaborative skills; ability to effectively and appropriately engage members of the care team, other clinic departments, leadership, and external community partners.
Exceptional communication skills; ability to effectively engage patients and assess needs and concerns to ensure that the patient is referred appropriately.
Excellent in-person and phone customer service skills; able to manage difficult and emotional patient situations and to respond promptly to patient needs.
Ability to adapt to change; approach challenges with an innovative and proactive attitude; and investigate resources.
Competent organizational and time management skills
Strong computer and electronic medical records skills.
EDUCATION & EXPERIENCE
Experience working with individuals with diverse educational, economic and cultural backgrounds; displays respect and sensitivity to cultural diversity.
Two (2) years' experience working in a healthcare setting such as medical clinic, hospital or other community health setting.
Experience working with people experiencing homelessness, mental, emotional and substance use disorders.
PREFERRED QUALIFICATIONS
Experience and knowledge of community healthcare
Associate's degree in Psychology, Sociology or Social Work.
