General Summary Works autonomously within scope of practice and credentials in providing and managing primary care medical services for adolescents under collaborative practice agreement with Medical Director. Reports administratively to the School Health Division. Responsible for initiation and maintenance of client services in the clinical setting in accordance with agency and Adolescent Health Center program policies, procedures and clinical standards. Performs comprehensive assessment, diagnosis and treatment of clients including ordering and dispensing medications. Counsels clients and families on health-promoting lifestyle choices and disease prevention. Collaborates with multi- disciplinary team members and outside providers, and makes appropriate referrals. Essential Duties Completes all responsibilities according to established protocols, policies, quality assurance, and best practices; and ensures regulatory compliance with HIPAA, OSHA, CLIA, and other relevant rules and regulations. Consults with Medical Director as required by collaborative practice agreement and established guidelines.Obtains complete and detailed health history, and reviews health records to develop comprehensive medical status reports.Performs age-appropriate and comprehensive physical examinations and records pertinent data in electronic health record. Completes sports physicals according to standards of care.Completes comprehensive assessment of health incorporating all dimensions including physical status, family functioning, basic needs, access to health care, and mental or emotional health.Orders laboratory, radiological and diagnostic studies appropriate for complaint and physical condition.Makes medical diagnoses and implements as appropriate immediate treatment or referrals to urgent care, hospitals, agencies, community resources, mental health professionals, or physicians.Institutes emergency measures and treatments or appropriate stabilization measures in response to injuries or medical or mental health issues. Coordinates transfer of care to emergency personnel or primary care provider as appropriate.Develops and implements care plans in response to assessments, test results, and findings. Evaluates and revises care plans as client status changes.Orders and/or administers medications per standards of care.Obtains and completes appropriate patient "Consent for Care" forms, ensures patient confidentiality and maintains orderly electronic health records (EHRs.) Follows established EHR policies and procedures as well as State and Federal laws for release of health care information.Participates in development and maintenance of protocols of services provided and procedures to follow when performing delegated medical acts.Counsels and educates clients and families on positive health-promoting lifestyle choices nutrition, and physical activity.Collaborates with clinic and agency staff and other community agencies and resources (e.g., physicians, local hospitals, social services, nutritionists, dentists, counselors, and so forth) through joint planning and coordination of activities, in providing comprehensive care.Evaluates immunization status and administers vaccines. Other Duties and Responsibilities Performs all other duties as assigned or in response to emergencies, workload, or staffing shortages.In response to public health emergencies, reports to assigned operations center for duration.Represents agency or program at community, public or governmental functions and forums.Keeps current on trends, developments, and best practices related to professional field and clinic services.Maintains up-to-date vaccines including flu (influenza,) in accordance with CDC guidelines: Recommended Vaccines for Healthcare Workers.
