Clinical Liaison Responsibilities:
* Provide an interface between health care providers and Traditions Home
Health Services.
* Develops and implements a program to contact all potential referral sources
within the hospital.
* Responsible for establishing, growing, and maintaining referral volume and
relationships with hospital staff.
* Communicates with existing referral sources and identifies new
opportunities within the hospital
* Delivers exceptional customer service, and assists with advanced care
planning discussions, and informative private home care consults for clients
and family members
* Responsible for ensuring the intake and referral processes are performed
effectively and timely to meet the needs of our clients, families and referral
sources
* Completes the admissions process on referred clients in emergency cases, as
staffing indicates, and for inpatient private home care admissions.
* Provides accurate and timely information regarding hospice services to
anyone inquiringReceives calls from referral sources, responds timely, and initiates
paperwork to begin Intake/referral process.
* Serves as client advocate in evaluating & developing plan of care.
* Offering in-services and education on the benefits of Private Home Care
Services.
* Provides an interface between health care providers and Traditions Home
Health Services.
