Accepts clinical responsibilities including, referrals; screenings/assessments, individual, group and family counseling, and crisis intervention. Serves military veterans, Reservists, National Guard, Active Duty, and military families with mental health and substance use needs. Provides clinical services for individuals to reduce the impact of their mental health symptoms and/or substance use on their recovery, and reduce the need for higher levels of care I.E. hospitalization, jail, etc. Provides supervision to assigned staff.The team works out of the Cobb Veterans Outpost, located at 840 Roswell Street, Marietta, GA. Duties and Responsibilities include:
Assesses immediate needs and assists individuals in identifying appropriate resources for additional assistance for meeting those needs.
Timely, compliant clinical documentation and billing practices following all HRBH policies and standards set forth by HRBH policy, including but not limited collaborative documentation, ensuring discharges are completed, etc.
Case management and clinical responsibilities including: accepting appropriate referrals; providing assessments, individual counseling, family counseling, group counseling, and crisis intervention according to the treatment plan; maintains a caseload.
Takes ownership for individual and referral source satisfaction by returning phone calls in a timely manner, communicating with referral sources with appropriate authorization, link and/or serve as advocate for individuals as needed.
Encourages individual responsibility and recovery by utilizing evidence-based interventions, providing comprehensive and ongoing assessment, referring individuals to appropriate internal and external services, involving and educating family, seeking and utilizing supervision, developing individualized and comprehensive plan of care based on the individual's strengths, needs, abilities, and preferences.
Completes all paperwork for individuals, as needed, while enrolled in services, according to policy and procedure, and grant requirements.
Works closely with the team members to facilitate and refer for wraparound services.
Educates community partners regarding programs and services available for individuals.
Attends community events to educate and make local citizens aware of the services provided. Assists with community outreach efforts.
Provides administrative supervision to assigned staff including coaching on meeting their job responsibilities and goals, monthly/quarterly/annual chart audits and staff evaluations, approving schedules/time off/timesheets
Communicates and collaborates with fellow leadership members to improve Highland Rivers service delivery.
Participates in managing, maintaining, and collaborating a cooperative service environment.
Attends all trainings as required by Highland Rivers.
Ensures compliance with DBHDD standards, CARF, HFR, and grant funding partners.
Interview, hire, and onboard new staff. Complete documentation for Human Resources
Other duties as assigned.
