Day Hab.- Assistant Coordinator (Healthcare)
Required experience and qualifications: High school diploma required. Some college credits are preferred. Valid New York State Drivers License required for all shifts.
Required Job skills: Successful employees will possess good interpersonal skills, be able to build strong relationships with people supported, care givers and families, and be very flexible. Practical skills include: computer use, reading and writing ability, the ability to follow instruction and direction, and the desire to grow with the program.
Job Responsibilities:
Supervision of the people supported Implementation of Program Habilitation Plans Assist in planning of activities Assist the people supported when needed including areas of self care, independent living and community integration. Transportation/Driving Vans Attend all mandatory training sessions including Orientation, Defensive Driving, First Aid, CPR and Residence specific trainings. Obtain SCIP and AMAP Certification 8. Complete all necessary documentation in Evero including: Outcomes Data Collection, Activities Record and Progress Notes on a daily basis; Assessments; Medication Records (if AMAP); Incident Reports, Seizure Reports and Behavioral Records when needed; and necessary correspondence with other Programs Correspond as needed with other Program Staff, Family members, Consultant Staff e.g. Behavior Specialist, Nurse, Nutritionist etc. Assist with developing the program schedule11. Any additional responsibilities for the successful operation of the program.
